To complete the application, start by printing or saving this page to your computer for reference as you continue through the application process. We recommend doing the same for the Checklist. NOTE: Information you input into the pages of this application are not saved automatically. Follow the instructions below to save your finished document to your computer. Conway Mykol Tax will need to receive your documents with your original signature including your Form 2848 which should be submited with your passport via DHL. |

| SHIPPING INSTRUCTIONS We recommend using DHL to ship your passport to Conway Mykol Tax at the address above. Make sure to arrange for return shipping of your passport back to you. Our turn-a-round time is typically 48 hours (depending on DHL schedule) from the time we receive your passport, perform a forensic examination of it, and hand it back to DHL using the return shipping label you provide. You can track your passport online at the DHL website. | ||
| PAYMENT INSTRUCTIONS: Prior to sending your Form W-7 ITIN documents to the IRS, you will be invoiced $400 USD via PayPal. Payment plans are available if needed. | ||
| SAVE / PRINT PAGE INSTRUCTIONS: Click on the 'Print this page' button near the bottom of the page. In the "Destination" section at the top of the PRINT dialog box use the dropdowm menu to select 'Save to PDF'. Click the 'SAVE' button at the bottom of the dialog box to open the SAVE AS window and choose where you want to save the document on your computer. To print a page follow your normal procedure for printing once you click the 'Print this page' button. |

| CLIENT TAX PORTAL: You will be assigned a tax portal for securely uploading, downloading, and reviewing documents associated with your application. It is a short term storage unit for your tax documents and will be available for conducting business with Conway Mykol for up to 5 subsequent tax years. A placeholder number (formatted simarily to an ITIN) will be assigned to you for using the portal only. Once you receive your ITIN, the ITIN will replace your portal placeholder number. Conway Mykol Tax will store your documents long term on a cloud for security reasons. Once your documents have been reviewed and downloaded by you, we will delete them from the cloud. | ||
